Before your first day, you will be given several forms which must be completed and returned before your child can start school. We've made these forms available as PDFs for downloading below. We are required by law to maintain each child's file with the following:

       Child’s Preadmission Health History
       Registration and Admission Agreement
       Physician's Report
       Identification and Emergency Information
       Consent for Emergency Medical Treatment
       Parent's Rights/Personal Rights
       Emergency and Medical Treatment Cards
       Earthquake Preparedness Form (Infants)
       Earthquake Preparedness Form (Preschoolers)
       Infant Food Service Plan (for infants only)

We also require a copy of your child's current immunization record for our files. It is the responsibility of the parent or guardian to keep this information current, including new addresses, home and work numbers, and immunization records.
